Your booking menu is an advertisement.

The TGA's position is that a public booking system letting a consumer select from a list of treatments involving prescription-only medicines, or showing prices for them, is likely to be an advertisement. That is a default booking page on most Australian aesthetics software.

Three problems, one cause

The booking system is the conversion, the exposure and the blind spot.

In Australian aesthetics the booking platform is doing three jobs at once, and it is failing at two of them. It is the conversion event, so the measurement depends on it. It is a public list of treatments and prices, which the TGA's guidance says is likely to be an advertisement. And it usually sits on the vendor's own domain, which is where the click identifier stops.

That is why fixing tracking and fixing compliance in this vertical are the same project. A clinic that rewrites its booking menu to remove prescription medicine names and prices is doing compliance work, and it is also doing the audit that reveals where its conversion data is going missing.

Fix the menu, then the measurement, then the value.

    Audit the public booking menu first

    Before any tracking work. If it lists prescription-only treatments by name, or shows prices against them, that is the exposure the TGA's guidance describes, and it is sitting on a page the clinic frequently does not think of as advertising.

    Establish where the booking completes

    Most Australian aesthetics platforms complete the booking on their own domain. That is where the click identifier is lost, and it determines whether a full conversion chain is available or whether you are working with hashed contact matching.

    Get a conversion firing on the completed booking

    Some platforms will send completed bookings to Google as conversion events using a conversion ID. Note the limitation: that has to be configured as a website conversion action, because imported and offline conversion actions do not generate an ID you can connect. So you get booking counts, not revenue.

    Attach a value that reflects the year, not the visit

    This is the step that changes the economics. A first visit worth a few hundred dollars makes the account look marginal against a $4.17 click. Published group data puts average annual client spend at $969, and the client returns across years. Bidding to first visit value is the category's central error.

    Reconcile against the practice system monthly

    Booking platform reporting and practice revenue diverge quietly, particularly where a marketplace commission applies to new clients. Checking them against each other is the only way to know what a new client actually cost.

The marketplace is also your competitor
At least one major booking platform charges an Australian marketplace commission of 20 per cent on a new client, with a minimum fee, while returning clients are free. So the platform monetises exactly the new customer flow your advertising is paying to create, and it lists competing clinics on the same page. That is not a reason to abandon it, and it is a reason to measure marketplace-sourced bookings separately from advertising-sourced ones rather than letting one take credit for the other.

Why do my cosmetic clinic bookings not show in Google Ads?
Usually because the booking completes on the software vendor's domain rather than yours. When the visitor crosses domains the original click information is dropped unless cross domain measurement has been configured deliberately, so the booking is attributed to direct traffic. Some platforms offer a Google Ads integration that sends completed bookings back as conversion events, which helps, though it typically has to be set up as a website conversion action and therefore reports counts rather than values. An Australian case study on one major aesthetics platform documented exactly this: appointments and their value were not reporting back, leaving the bidding without a signal.
Is our online booking page a compliance risk?
Potentially, and it is the risk most clinics have never considered. The TGA's guidance is that publicly available booking systems which draw consumers to a service on the basis that specific therapeutic goods are used are likely to be an advertisement, and it names two fact patterns specifically: a form where the consumer self-selects from a list of treatments involving prescription-only medicines, and showing price information. A standard aesthetics booking menu listing named injectable treatments with per unit prices matches both, and it usually sits on a third party domain the clinic does not control.
What conversion value should a cosmetic clinic use?
Something closer to annual value than to first visit value. The published figure we would anchor on is average annual client spend of $969, disclosed in a listed Australian clinic group's results, against a category average cost per click of $4.17. A clinic bidding to a first visit worth two or three hundred dollars will conclude paid search is marginal, and will be wrong, because the maintenance cycle in this category runs for years. Government guidance puts toxin duration at up to six months and hyaluronic acid filler at about a year, so the repeat cadence is slower and the relationship longer than the industry's usual three to four month framing suggests.
Can we use Customer Match to retarget our client list?
No. Google treats Health as a sensitive interest category, and advertisers promoting in those categories cannot use advertiser-curated audiences, which rules out Customer Match, similar audiences and anything built from an uploaded client list. This surprises a lot of aesthetics clinics, because the category feels commercial rather than clinical and the tactic is standard in adjacent retail industries. Measurement uploads are a different thing and remain available.
